§ 13.20.340. Signatory and certification requirements.

  • All permit applications, reports and plans submitted to the City by industrial users pursuant to Sections 13.20.270, 13.20.272, 13.20.305, 13.20.310, and 13.20.330 shall be signed and dated by an authorized representative of the user. The signature shall accompany the following certification statement:

    "I certify under penalty of perjury under the laws of the State of California that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations."

    Analytical reports submitted directly to the City by a certified analytical laboratory at the request of the user for samples of wastewater collected at user facilities may be signed, dated, and certified by the laboratory manager in lieu of an authorized representative of the user; however, such reports shall be accompanied by a statement, signed, dated, and certified by an authorized representative of the user, as above, which verifies that the sample identified in the analytical report was collected on the date and time indicated at the location indicated, and using the method indicated on the analytical report. This signed, dated, and certified statement may be included as part of the chain-of-custody form for the sample.
